To provide Service Coordination services to individuals with who are currently enrolled in Pennsylvania Waiver Services. Service Coordinator will be stationed locally in Pittsburgh. This position requires traveling, so a valid Driver’s License is required. Service Coordinator will be responsible for 40-50 consumers. Candidate must be flexible, have the ability to self –supervise and maintain consistent communication with superiors. Time management and organizational skills are essential. A service coordinator candidate shall meet the following criteria: I. Have a Bachelor's degree including or supplemented by at-least 12 college level credit hours in Sociology, Social Welfare, Psychology, Gerontology or another behavioral science. Or a combination of experience and training that adds up to 4 years of experience, and education which includes at least 12 semester hours of college level courses in sociology, social work, social welfare, psychology, gerontology, or other social sciences. (A) Experience includes (1) Coordinating assigned services as a part of an individual’s treatment plans. (2) Teaching individual living skills (3) Aiding in therapeutic activities (4) providing socialization opportunities for individuals.
A service coordinator must finish 40 hours of training within the first year of employment with an additional 20 hours of annual ongoing training on the following:
o Conducting a person-centered assessment.
o Developing and modifying a participant's service plan.
o Improving communication skills.
o Utilizing conflict resolution skills.
o Completing documentation.
o Understanding the disability of a participant served.
o Utilizing the departments data systems
